Healthcare matters. But many of the factors that shape our health are found beyond the walls of healthcare itself — in relationships, communities, everyday life and the support systems surrounding us.
At Norrahammar Primary Care Centre, a new pilot inspired by successful models in Alaska is exploring how continuity and trust can be restored in an increasingly fragmented healthcare landscape. Through the “Invånarteam” (Resident Car Team) initiative, residents are connected to a small, dedicated team of professionals who know them, follow them over time and remain their primary point of contact throughout their healthcare journey.
The Learning Hub also includes visits to the Family Centre and the social enterprise Mina Minsta at Bruket Norrahammar. Together, they demonstrate how healthcare, family support and community initiatives can collaborate across organizational boundaries to support families facing economic hardship, social isolation and everyday life challenges.
